Requirements to cheer for Novato Youth FootbalL


Due at Sign-up:
1. Copy of Birth Certificate (New player only)
2. Copy of Insurance Card

Due by August 3rd for cheer (First day of cheer practice):
1. 2009 Player-Parent Contract
2. Adult Code of Ethics
3. Birth Certificate (3 copies)
4. 2010 Medical Release Form (signed by a licensed medical professional)
5. 2010 Physical Fitness & Medical History form
6. 2009-2010 end of year report card (3 copies)
7. Medical insurance card (1 copy – front and back)
8. Equipment & Uniform Maintenance Agreement

Cheer Cost:
$300.00 (includes Cheer camp (4 days) & competition entry fees)

Practice is 5 days a week thru Labor Day, then moves to 3 days a week; the first week
of practice is known as "conditioning week" where players complete 10 hours of conditioning and must be completed before players can participate in contact drills.
